Bought it new from a Verizon corporate store.

Doesn’t matter. Apple support is free for the life of the product, and they will honor the warranty. Contact them ASAP.

This happened unfortunately with 2 of the 3 cameras on my old 12 Pro. Apple will repair it if you’re under AppleCare. Their fix is to replace the entire camera module which is around $500 if you’re not under warranty. They said sometimes the tiny camera wires come apart from the motherboard and there is no way for them to repair.
YMMV but when I got this issue, I was able to circumvent it by using a third-party camera app. Which proves this is a BS issue, I know, but I’m just the messenger.
How old is it? Apple will often repair or replace an iPhone (even out of warranty) if there is a known fault or out of courtesy. I had this done with an iPad Pro which was out of the warranty period.

Yes, OP should go to Apple ASAP.

It’s not necessarily saying the camera’s definitely been replaced, it’s saying it’s unable to tell that it for sure hasn’t been. Likely the 1x camera has failed. If the device is within its 1-year warranty you can get it fixed for free as long as there’s no damage.
